Admission Process
Patient who need treatment on Insurance card/Employer referral letter are advised to contact Corporate Admission Office located on first floor of Mansoorah Teaching Hospital. Patients can contact corporate admission office on 04235419510 (Extension 156) and 0300 8440355.
If you are advised hospitalization and have a valid insurance card, please follow the following steps.
General Patients
- Show prescription to Advice Admission.
- Complete Name of Patient (Name, Father’s Name, Ph No & Address).
- Patients & Attendant CNIC copy.
- Select categories (General, Semi Pvt & Private Room) according to facilities.
- According to rate & facilities select category.
- According to procedure to pay complete payment billing department.
- Submit payment on billing department and get official receipt.
- Admission time get complete information to billing department.
- Submit payment and Come to Qazi Block or Gynae block for admission & get file after start treatment.
- If any issue or query call to Finance Manager (0321-4042583) or Deputy Medical Superintendent (Admin) for any assistance.
Corporate Patients
Present your insurance card and CNIC at Corporate Admission Office. Corporate Officer will admit you to the hospital and intimate insurance company. If case is approved by Insurance Company, you can get treatment up to the amount approved by insurance company. If your case is rejected by Insurance Company, kindly pay cash before discharge.
- Visit Corporate Admission Office with your insurance card and CNIC and collect insurance intimation form.
- Ask Physician/Surgeon to mention the medical necessity for hospitalization on Insurance Intimation form.
- Submit the form at Corporate Admission Office. Corporate Officer will intimate insurance company and admit you to the hospital after approval from Insurance Company.
Note: If your organization has health care agreement with FMH, obtain referral letter from your organization and visit Corporate Admission Office. Corporate Officer will admit you to the hospital after necessary documentation. In case of emergency, patient will be admitted on employee card only.
